Report Fraud, Waste, and Abuse

OIGH accepts reports involving suspected fraud, waste, abuse, improper payments, misuse of public benefits or funds, and related misconduct affecting Maryland health programs. Reports may be submitted anonymously. OIGH reviews the information received and determines whether the matter should be investigated, referred, or otherwise addressed.

What to Report

You may report concerns involving:

  • Overbilling, false claims, or improper billing by a provider or pharmacy
  • Improper payments or misuse of public funds
  • Recipient eligibility concerns
  • Misuse of benefit cards or public benefits
  • Fraud, waste, abuse, or misconduct involving a Maryland health program
  • Other suspected misuse of State health program funds or resources

Information to Include

Provide as much information as possible, including:

  • Names of the individuals, providers, organizations, or programs involved
  • Dates and locations
  • A description of what occurred
  • Dollar amounts, if known
  • How you became aware of the concern
  • Supporting documents, records, photographs, or other evidence

You do not need to have every detail before submitting a report. Clear and specific information will help OIGH review the matter.
